Default files showing up twice in search results

I've couldn't find an answer to my question else where so I'll ask it here
in
hope for an answer.

When doing a search for files , results are duplicated only if the file is
located in "My Documents" folder. The type of file doesn't seem to matter
ie...mp3, txt, doc

I've inspected each duplicate to see if maybe there is a difference, but
they are the same file. (name, size, , location, even date modified)

Any help appreciated

It's merely showing both paths to the same file.
