It seems that at some point the sensitivity of mouse position has gone
through the cieling.
That is, I now almost have to hold the mouse with both hands to make sure
the second click in a double click comes out in exactly the same place as
where the first click was. Otherwise it doesn't take. Even a pixel of
movement from first click to second click seems to kill it.
Is there any place to set this?

You can set mouse sensitivity in the Mouse applet in Control Panel. Some
branded mice such as Logitech, etc. have their own drivers and sometimes
their own Control Panel applets. Since you didn't tell us anything about
your mouse, that's as specific as I can get.
So:
1. Try adjusting the sensitivity from the Control Panel applet.
2. If that doesn't help and you have anything except a generic mouse,
download the latest drivers from the mouse mftr.'s website.
3. If #1 and #2 don't work/apply, then try a different mouse. Does the
problem still happen?
And of course:
The First Question Of Troubleshooting: what changed between the time things
worked and the time they didn't?
Adding to Malke's reply. If it is a cordless mouse,then have you checked the shape of the batteries in the mouse or in the receiving unit??

It's an HP M859PU - a basic, corded optical. It didn't come with a driver
disk.
I couldn't pinpoint onset of the issue if my life depended on it.
Before that I had a Microsoft mouse of the same description. I threw that
away at about age 6 months when the pointer started careening wildly
across the screen. (Usually it would jump from where ever I was working
to the upper left corner of the screen.)
I very much like this general type of mouse (corded optical) for my home
based laptop and would welcome suggestions for like replacements.
(Wireless seems to be taking over the world!)

Thanks. I gave up on wireless. With the receiver high on the left and
the sender low on the right (desk pullout) the reliability was definitely
insufficient.

I got another Microsoft corded optical. It was less position sensiive,
but it was slow as the seven year itch and there was no speed setting for
it. I went back to the quicker mouse - which also had a fraction of the
drag on the mouse pad.
