Vista x64 Going nuts after: Update for Windows Vista for x64-based Systems (KB941649)
I applied this patch:

Now my Explorer (only when the window is focused, not when the window is
minimized) is going nuts,
In Process Monitor 20% (always 20%) of all the events are only these ones:
class: registry
operation: regquerykey
result: success
path: HKCR\CLSID\{0AFACED1-E828-11D1-9187-B532F1E9575D}\shell
and with nuts i mean crasy, my dual core 6600 is contantly on 96% load
in less then 8 minutes i have 26.000.000 events of which 20% are descibred
as above.

Have you tried reverting the patch off (use System Restore via booting from
media) and then reapplying the patch with A/V disabled?
