Default Creating folder for downloads

I will be dang if I can figure out how to create a new folder, when
downloading, and am the file save window.
Right now I just download the file, and then go create the new folder, and
move the file to the new folder. One step that I shouldn't have to do.
Any suggestions?

Try hitting the details/more button (if you're seeing the abbreviated save
to dialog) RIGHT click and choose new/folder.

There is also a create new folder icon in the tool bar
