If you have a dual core processor, it shows the second CPU core.

On the CPU, sidebar item, what does the smaller right meter Show? Never seems
to change much.

I have a dual core processor the smaller second meter shows RAM usage.
You can single click the meter then hover the cursor over it will let
you know what they are. I am assuming you are both talking about the
default CPU meter that comes with Vista.
If there is a setting to change the second meter to show core2, please
reply, I see none.
If you mean the gadget that has the 2 dial type meters, if you left click
the right dial, a pop up description will appear and show "Random Access
Memory (RAM". So this is showing how much of the installed RAM is being
used.
