Default Bootup Error "cannot determine file system of drive \??\volume{hex

This is error is driving me crazy. My machine crashes on the first boot but
works fine after I restart it and tell windows to start normally when the
option to go into safe mode. The error reads as follows (the error is only
visible after turning on “OS boot information” under msconfig) :

"cannot determine file system of drive \??\volume{hex}"

I have tried the following:

CHKNTFS /X C:
repair system from windows vista installation disk
CHKDSK /f /r (from the windows vista installation disk via the command
prompt)
Bootrec /Fixboot (from the windows vista installation disk via the command
prompt)
Bootrec /Fixmbr (from the windows vista installation disk via the command
prompt)
Bootrec /RebildBcd (from the windows vista installation disk via the command
prompt)

Any Other ideas?

SpinRite
Reinstall OS

Any help will be appreciated

Back up essential files and replace the hard drive.

Run a hard drive diagnostic program.

Run a memory diagnostic program.

memory dianostic turned up nothing

no luck with the hard drive diagnostic

in the process of backing up and replacing hard drive

Will report back later.....
