Is there a template for perspective members to fill out

I am president of Conservative Student Union at my college, I need a form for
people who are interested in finding out more about our club and its events.
I aslo want to know what their related interests are. We desperately need to
recruit new members or our club will disappear.

As long as you're 'Conservative', no matter what you do, you will disappear!
