looking for a template to give const. crew, with jobsite info

I am looking for a template or a way to come up with a cover sheet that would
have information for my crews as to what there job is, where is the job, when
is their deadline date and any misc information I need to put down to help
them with their job I send them to

Couldn't you just make s simple table?
