Why are they so hot? Would it affect its efficiency?

I've installed several of them and never known any of them to get more than
warm. What is it powering and what's the airflow through the case like?

Same here. I bought a TruePower II 380 to power my AMD 939 system
and now it's being used to power my Intel 775 system, both with a
BFG 7950GT/512 MB video card. Very little air comes out of the power
supply and it's no more than warm.

Efficiency, no. Can air move through it -- does the underside over
the CPU draw from 80mm fan and exit from a 60mm in the back? Ever
opened one up to find it full of yellow caked-on dust?

Air flow should be fine. Ambient temperature is
about 28 degree Celsius. It's a Asus M2N-E
motherboard using AMD X2 3800+ CPU, 4 sticks
of DDR2-800 RAM.
It's not very hot, but it's not definitely
not as cool as another cheap 350W power supply.
