I have a Philips DVP 642 DVD player connected to a TV set that has only
mono sound, and only one audio input jack.
Is there any way to mix the audio output from the DVD player so that the
single audio channel on the TV receives both left and right audio channels
from the DVD player?

I believe some DVD players can do this from within the menu. Otherwise
get yourself an RCA Y-adapter:
